Santa Clara Valley Healthcare’s contract with Anthem Blue Cross is ending. As a result, they will no longer accept Anthem Blue Cross health insurance after December 31, 2025. VMC is already not accepting some specialties now.

How do I switch from Anthem to Santa Clara Family Health Plan?
- Please call Health Care Options at 1-800-430-4263 (toll-free) and ask for “Fast Disenrollment”. Spanish Line 1-800-430-3003
- Monday through Friday, 8 a.m. – 6 p.m.
- Ask to switch to Santa Clara Family Health Plan
- Your new health plan will be effective on the 1st of the Month
- Make sure you have your CIN number
- Ask to keep one of your School Health Clinics as your primary care provider and provide your clinic’s NPI
