However, Valley Medical Center and Anthem have reached a contract extension and secured a long-term agreement, ensuring continuity of care for our community.

School Health Clinics will continue to accept Anthem Blue Cross for all primary care services until then. If your access to specialty care is affected, our insurance enrollment specialists are available to help you review your options and update your coverage as needed.

How do I switch from Anthem to Santa Clara Family Health Plan?
- Please call Health Care Options at 1-800-430-4263 (toll-free) and ask for “Fast Disenrollment”. Spanish Line 1-800-430-3003
- Monday through Friday, 8 a.m. – 6 p.m.
- Ask to switch to Santa Clara Family Health Plan
- Your new health plan will be effective on the 1st of the Month
- Make sure you have your CIN number
- Ask to keep one of your School Health Clinics as your primary care provider and provide your clinic’s NPI
