Patient Navigator & Insurance Enrollment
We are committed to removing barriers that prevent patients from receiving the care they need. We help patients access outside providers when specialty care is recommended, and advocate for patients to ensure their voice is heard. We also help patients enroll in healthcare coverage like Medi-Cal, Medicare and Primary Care Access Program (PCAP).
Behavioral Health
We provide mental health screenings, in-house counseling, and referrals to specialty providers so that patients have access to comprehensive behavioral health services.
Health Education
Our program teaches children and their families about the importance of nutrition and physical activity so they can live long and healthy lives. Multiple health-related subjects are covered like promoting growth and development through healthy behaviors and disease prevention.
